My work as a clinical social worker with the University of Maryland Medical Systems and Johns Hopkins Medical Services Corporation included extensive involvement in dealing with patients suffering from chronic and/or terminal illnesses, including kidney, pancreatic, and cardiac conditions. I ve dealt with pre and post-transplant patients, and a part of my mental health treatment plans required a knowledge of the physical and mental effects of the various medications and medical treatments on the psyche of the patient. Side effects of medications can include emotional and mental changes; they can also include physical changes, ranging from pain to weight gain to loss of mobility or cognitive functioning that can lead to depression, anger, suicidal thoughts, and other emotional/psychological reactions. On top of this, the physical and emotional condition of the patient can have similar effects on family members - depression, guilt, and anger are all common. Families can also turn to drugs and alcohol. All of this needs to be taken into consideration when developing individual and group/family therapy and treatment plans.

 

During my time working in two Army Medical Centers, the veterans, returning soldiers, and ready-to-deploy soldiers presented common and uncommon situations that often included medical treatments and pharmaceuticals/medications. Besides the medications normally associated with common medical conditions and physical issues (antibiotics, steroids, pain medications, diabetes meds, meds for cardiac conditions, etc.), there are the effects of psychotropic pharmaceuticals, medications used to wean patients off illegal drugs, and the medications used to treat medically unexplained conditions. One must also consider the interactions of prescribed pharmaceuticals with ones that patients may have taken - or might still be taking - illegally, which can lead to serious physical and mental/emotional adverse reactions.

 

At the Dwight D. Eisenhower Army Medical Center, one of my responsibilities was working with multi-disciplinary medical teams to coordinate patient services. I also conducted daily training and periodic in-services for interns, residents, and physicians, and referred patients to primary care physicians. 5.  Knowledge of acute and chronic psychiatric disease categories, established psychiatric theories and treatment ; skill and ability to apply treatment modalities in working with emotional and behavioral disorders, and knowledge of psychotropic drugs, their expected results and possible side effects.

 

In my graduate program for my Masters in Social Work, I carried out extensive studies in psychiatric treatment modalities and theories. Among my training, I completed courses in Social Policy in Health Care, Human Behavior II, Health and Illness, Evaluation Research, Paradigm Clinical Social Work, Family Therapy, Clinical Social Work, Physical Illness, Clinical Social Work Internship, and Psychopathology: Addictive Behavior Patterns. I ve also completed continuing education in a wide variety of areas, such as Substance Abuse, CPR, Nutritional Therapies, and Complicated Trauma: Diagnosis & Treatment.

 

During my time at two Army Medical Centers, as well as the University of Maryland Medical Systems and the Johns Hopkins Medical Services Corporation, I have evaluated and treated a wide variety of psychological, emotional, behavioral, and physical disorders, including the following: family issues, abuse, fear of terrorism, victimization, delinquency, medically unexplained syndromes, crisis intervention, behavioral health conditions such as PTSD and chemical dependencies, violent or high-stress incidents, maltreatment, youth issues, language disabilities, tic disorders, over-activity, impulsivity, inattention during social interaction, developmentally inappropriate social skills, cognitive skills issues, neuronvegative signs, eating disorders, depression, violence, abusive behavior, suicidal tendencies, self-destructive behavior, delinquency, vocational concerns, neglect, poor child-parent attachments, and chronic medical and mental health issues such as transplants and cardiac conditions.

 

All of the above conditions and issues have required extensive bio-psychosocial assessment, health screenings, and working with health care professionals to develop complex, specialized treatment plans. Many of the conditions I ve dealt with have involved patients prescribed psychotropic drugs, or who have taken psychotropics without prescription.

 

I m familiar with a comprehensive list of treatment modalities for emotional, behavioral, physical, and psychological conditions, and I have consistently maintained compliance with all facility and industry regulations, as well as the Diagnostic and Statistical Manual for Psychiatric Disorders principles and practices.

As a Social Worker and Clinical Social Worker, and also a Health Management Support Professional, I am constantly communicating with health care professionals, as well as with other individuals involved in the care of patients, including other social workers, physical therapists, and staff members from various agencies and community programs, including home health care organizations, adult and child protective services, law enforcement officers, patient advocates, insurance companies, substance abuse treatment facilities, and military/civilian community services groups. In all the positions I ve held, I ve been responsible for coordinating post-discharge ongoing treatment plans and arranging consultations and services for patients with appropriate groups, based on the patient s needs.

 

At Dwight D. Eisenhower Army Medical Center, I work with multi-disciplinary medical teams to coordinate patient services. I refer patients to, and coordinate with, primary care physicians, Veterans Administration, and community agencies. I have obtained durable medical equipment, set up home health care services, and overseen patient transfers. I interact extensively with Tricare, Insurance Companies, Crimes Against People, Army Community Services, Family Advocacy, Adult and Child Protective Services, and other organizations and agencies to manage benefits and provide necessary services.

 

At William Beaumont Army Medical Center, I coordinated care with military, civilian, and community agencies, obtained durable medical equipment and arranged home health care services, and managed benefits with providers. I also served on the Trauma Committee and JACHO Advisory Committee.

 

With the University of Maryland Medical Systems and Johns Hopkins Medical Services Corporation, I participated in multi-disciplinary rounds and team meetings, assessed the need for community services for patients, and conducted crisis intervention and bereavement counseling, individual and family therapy, and patient/family education. I also carried out discharge planning, outpatient case management, and reporting of maltreatment, neglect, and assault cases. I worked and communicated extensively with the police, CPS, APS, foster care groups, and other organizations.
