In this long-term, multi-faceted assignment, was charged with comprehensive caseload management concerning securing support services for up to 130 children and their families.  This included working with service providers and community resources, and addressing a variety of needs, including dental and medical care, education, daily child supervision and multiple rehabilitation services for eligible children as well as entire families.  In this broad-based work, developed interaction skill sets needed for working effectively with many personalities and served as the point-of-contact for families and support organizations.

 

At the outset of providing service to a child/family, performed initial assessment and evaluation processes, including verifying facts, tracking all applications and approving all transactions.  This included interviewing parents/guardians, analyzing all income and financial information and developing determination of eligibility for assistance as well as assistance level.  In a one-to-one setting, provided explanation to families regarding variety of programs availability, eligibility requirements for the different programs, and the responsibilities of the family involved.

Once eligibility for services was confirmed, established close working relationship with parents to determine areas of family requirements to be addressed, and setting goals to be met by families as well as agency and other support structure segments.  In support of these goals, served as a source of guidance, objective insight and as the go to person for answers regarding any specific technical issues.  Utilized a personal, hands-on approach to children and families involved in programs, conducting follow-up home visits to assess the progress of the assistance program and to ensure each case was being handled according to program directives and that it was meeting performance goals.

 

Additionally, conducted specific training seminars for family members to assure they were well-informed regarding preparing/adhering to family budgets, and availability of classes on parenting skills, medical/dental care, child care, educational resources, substance abuse rehabilitation and crisis intervention.

 

Was actively involved in Head Start program for youngsters, conducting outreach education and other activities to involve children.  Researched various areas and developed planned programs that supported and actively promoted the Head Start philosophy and goals.  Researched content and availability of community resources already in place and worked closely with all organizations to assure that all children and families enrolled in program received health, educational and rehabilitation services as set forth by Head Start in their performance standards.  Additionally, personally conducted needs assessment for community to identify available family support services and target population service requirements to be addressed.

 

Administratively, in coordination with Center staff, management personnel and involved agencies, ensured that all policies and services, in place or planned for future involvement, fell within the parameters of being legal, were logical, workable and desired by clients, and were possible to achieve.  Actively developed and facilitated plans for long-range goals designed to improve all Family Advocacy programs.  As necessary, proffered recommendations for changes to policies and/or procedures so as to improve service delivery within directed timeframes.  Was tasked with ensuring activities followed established policies and procedures and met program performance standards.  Additionally, dealt with difficulties encountered in programs regarding Quality of Life.

 

When necessary, reported any instances of spouse and child abuse or neglect to TDFPS state agency.   Authored reports, memoranda and case notes, documented/updated all records and charts regarding case progress, and recorded all adjustments, interventions and referral actions.  Maintained confidentiality of all records, files and information, and consistently met target goals and deadlines while providing comprehensive excellent case management.
